Carefully lol!

I will say it's easier for someone else to do it but you can do it by yourself.

Is it a metal speculum or dilator? With speculums lube yourself and the device well...slide it in carefully but not completely to the screw.

(This could nick your arsehole as you move it)...

Reach around and gently work the screw. You will feel it stretching..just go as far as you feel comfortable with.
