i believe i first read that advice in "The Happy H**ker", a book by Xaviera Hollander. In her memorable words, "he can just fuck away", although i'm sure it took a lot of practice to reach that level.

When you first try you will need to gently push over a bit of resistance to get a big cock into your esophagus. squeezing yr thumb in a close fist is supposed to help. I just think it is distraction.
