The remainder is about blood flow and a less effective flow to the penis because of vascular issues some of which are lifestyle (itself often weight related - think type II diabetes) but may have other root causes eg heart condition.

In the end it is what you do with it not how big it is but that isn’t necessarily an easy fix.
