Serious question.

Do the people who use the word clean to describe their sexual health ever consider how the use of that word affects anybody, like myself, living life with an incurable virus, or similar?

No, I don’t think they do. I assume ignorance rather than malice. I think some of them also perhaps use it as shorthand for “clean bill of health”, which is slightly less insensitive I feel

I always thought it ment no smeg and knows how to use soap and water . Remember a shower is for life not just for Christmas

I always find it weird when people use the term clean when refering to STI status, like somehow having an STI makes you dirty. I don't actually think they give it any thought to be honest.

Also I sometimes assume people use it refering to personal hygiene instead.

I prefer to use the term negative when referring to STI status saves any confusion.

I do see it used plenty in regards to personal hygiene, more often than not though unfortunately it’s in regards to status.

I had this conversation once with somebody, before they knew my status, who tried to backpedal so fast once I told them about some of my experiences… it was almost funny
